REQUIRED ATHLETIC FORMS

State law requires all student-athletes to have current sports physicals (this is different from the school physical form) and consent forms on file with the school before a student may practice or play in a sport. These must be on the FHSAA-issued forms. No other forms can be accepted.

Steps to obtain athletic clearance: 

  1. Go to: https://athleticclearance.fhsaahome.org/index.php and create an account or log-in if you have a preexisting account
  2. Upload the entire athletic physical completed by a physician must be on the FHSAA EL2 form All students that will be participating in sports MUST upload the EL2 (Physical Evaluation) and digitally sign the EL3 form (Consent and Release from Liability) on AthleticClearance.com PRIOR to starting conditioning/tryouts. Forms are available in the school and can accessed by clicking on the links below. You must present this form to the Physician at the time of the Office Visit.   3. Complete the following NFHS Videos: Heat Illness Prevention, Sudden Cardiac Arrest, Concussion for Students & Sportsmanship (See detailed information below on how to complete these short video courses)

How to Access Videos 

 Go to “nfhslearn.com” 

  1. Click “Register” in the upper right hand corner - set up your account; must be your STUDENT NAME for the account 
  1. Click on “Courses” on the top banner 
  1. Find course you are taking: 
  2.     - Concussion in Sport
  3.     - Heat Illness Prevention
  4.     - Sudden Cardiac Arrest 
  5.     - Sportsmanship
  1. Scroll over the picture and click “View Course” 
  1. Select your State 
  1. Click “Order Course” 
  1. Choose “Myself” & click “Continue” 
  1. Click “Checkout” 
  1. Check the Agreement box 
  1. Click “Continue” 
  1. Click the words “Click Here” at the end of the first sentence 
  1. Click “Begin Course” 
  1. You must go through all the prompts throughout the video. Each video has chapters and sections within the chapters that must be completed. There will be a short quiz as well      Upon completion of the course DOWNLOAD & SAVE your certificate in PDF format. 

 


This set of forms is IN ADDITION to the School Entry Health Exam and updated copy of the Immunization Record – required by State Law for all NEW and TRANSFER students entering CCCHS.
