Re-Enrollment Procedure for  Current Students

The following list will help guide you through the re-enrollment process.  It is a good idea to print a copy of the list. Read the instructions carefully, fill out all forms completely and turn in all required documents to avoid delays.

Re-Enrollment Instructions/Forms

  • Student official class schedule will be mailed prior to the first day of school if all paperwork has been completed and submitted.


Re-Enrollment

  1. Re-Enrollment Contract (user name & password, must be completed for each  returning student): Click on Re-Enrollment Contract link. Complete this contract on-line. You will be asked to enter your credit or debit card in the discounted non-refundable fee of $500 if paid before March 23, 2010. After that date the fee will return to the regular non-refundable fee of $625. Be sure to make a copy of your payment receipt.
  2. SMART Tuition Agreement Form, Download the on line SMART Tuition Agreement Form,complete it and return to the Admissions Office.
  3. 2010 - 2011 Parent Involvement Contract: Download the on line Parent Involvement Contract, complete it return to the Admissions Office.
  4. Car Pool Information : (user name & password required): If you wish to participate in our car pool program, fill out the on line form  This information will come directly to the Admissions Office. We will provide the car pool coordinator with your information. Closer to the start of school we will advise you of whom to contact.

Cost of Education Partnership  (COEP)

Our 2010-2011 “Cost of Education” Partnership reflects our shared commitment to providing the very best in a Catholic education for your child. Clearwater Central Catholic High School remains steadfast in providing a quality education centered in gospel values and dedicated to the development of the whole person.

This partnership highlights our efforts to further the mission of our school by investing in your child and by inviting each CCC family to partner with us toward the actual cost of their child’s education. You will receive by mail the Cost of Education materials in March. It is required that you return your commitment form, whether or not you choose to participate, to complete the re-enrollment process.


Other Forms Due before July 15, 2010

If your student will be participating in any sport activity, including try outs, conditioning or practices, the FHSAA Rules require that these two forms be on file with the Athletic Director before any physical activity can take place.

These forms shall be valid for a period not to exceed one calendar year from the date of the practitioners/parents signature.  It would be advised that both forms be completed during the summer time, so your student-athlete does not have any eligibility problems during the academic  year.   


You will receive in the June mailing the following (these are due July 15, 2010)

  • Combination Form (treatment consent form)
  • Clinic Card
  • Textbook Credit Voucher

Official schedules will be mailed to families who have returned all required forms before the start of school.
